Spoiler: Straight Jacket 3.0 

Background: Long story short: A once very skilled freelancer agent capable of almost every form of challenge. His employer sought out a rare gas founded by a team of other fellow agents. This employer tricked Sly into thinking this equipment would help his physical side thus making him more of an effective and efficient worker. The gas didn't perform as planed and he developed a advanced type of MPD and complete memory loss.

Personality: These personality call themselves 'Personality fragments' or Fragments for short. They each represent a different emotion/function.

Spoiler: Psycho 

The main fragment and seen as the most former of the group and often referred to as the host. Psycho has the most control; being able to call out whatever fragment(s) he wishes, though he's not exactly the brightest. Psycho represent joy and optimism and speaks in a higher tone when he's happy or excited but his 'serious' tone can be confused with a jokingly serious sound. Psycho can can be taken as the youngest most colt like fragment.

Spoiler: Loco 

Apathy. Bitter and self centered, Loco has a love for food and sleep though he does show interest with things he thinks are worth "all the effort" or self entertainment. Loco is quick to make a sarcastic comment even at things that he'll be quick to regret. He has a knack for finding any way to get out of doing a job, he often tricks Psycho into doing it for him

Spoiler: Crazy 

Rage. Crazy is a typical villain with typical villain dreams and goals, though he's all bark and no bite. Crazy speaks with a slight British accent and has an unhealthy tendency of making threats (silly) and seeing everything in a negative light. He is known for his overuse of 'evil' words and a maniacal loud laughter

Spoiler: Biz (Bizarre) 

is the organization of the family. He shows high liking to physical labor and simply getting stuff done. He is easily the oldest (mentally) of the fragments as depicted in his verbiage and general beliefs. Biz speaks in a gruff and commanding voice and has a comedic hate-hate relationship with Loco as Biz seeks to give Loco the dangerous jobs as he is worth any risk. Biz cant seem to comprehend the fact that if Loco were to 'die' so would he.

Spoiler: Tic (Lunatic) 

Tic can easily be confused as a hate fragment though he is in fact Deceit. Always trying to find ways to fooling any 'Air Breather' he can. Tic seems to be the only one capable of accurately imitating the other Fragments' voices, mannerisms and and overall personality

Spoiler: Mental 

The dark side of Psycho's mind. Mental is Hate and Fear. He almost never talks and rather uses grunts, growls and groans as a form of communication. While Mental may be the 'fighter' of the pack, he isnt exactly hostile though he is extremely defensive and short tempered. Some fragments use Mental as a defensive means and try to stay on whatever good side he may have

Spoiler: D (Demented) 

Easily the most intelligent fragment. D is Logic and constantly working even though he may not be present. He is obligated to assist any who Psycho would call friend. D speaks in monotone voice and to note that D is the only fragment who cant just 'appear'. He must be requested by anther pony or another fragment.

Spoiler: Recollection 

Recollection is the only 'fragment' who is awake when SJ's mask is removed. Recollection only remembers fuzzy memories of his past and is the closet personality to Sly Fathoms. Psycho and Co. have no connection with Recollection and vice versa therefore Recollection could be the holder of any long term memory. No fragment likes to have his/their mask removed as it is a equal of blacking out.

Cutie Mark: His entire life, Sly Fathoms was a blank flank. That was until the day the mask was donned on him. He was meant to be a holder of multiple personalities. No-one can claim they know why exactly.

Use: Shout box. And maybe Forum.

Other: NOTES:
White Eye: Biz's specialty allows Psycho (And Biz himself) to have every fragment out at once though at the cost of serious mental drain.
Red Eye: Mental's specialty. Basically adrenaline and strength enhancement for a short time.

Only 3 fragments can be present at once (unless White Eye). A random 'head twitch' indicates the fragment(s) switching randomly. One minute it could be Psycho, Loco and Crazy. The next minutes could be Biz, Loco and Tic.

Unfortunately some emotions didnt make the cut, this however, does not mean they cant be created by showing Psycho acts of this emotion.
Keep in mind that some didnt make it for reasons that might be deep inside his maze of a mind.

NOTE! I spoke to the RP mods and SJ was cleared (months ago), I was only needed to see how my RP skillz were. Seeing as how SJ is my main OC, I thought I'd try again.

We've had a look at this, but we do not believe we could accept this without a number of changes which would most likely destroy your original idea completely. These changes include, but are not limited to, changing appearance, skills, abilities and personality.
Jacket looks far too out of place, you could not possibly put him in the show without clashing with the rest of the show. You would at least have to drop the entire gasmask-gear, including the tank.
The backstory is short at best, and doesn't tell us anything about the character, where he comes from, his family, or anything beyond that he was tricked into this. What we do know of his history also clashes with the tone and known facts about Equestria, and we could quite honestly not see Celestia allowing a company like that of his boss' to continue.
The numerous personality-traits also seems rather unsuited for the show, and while we have seen a mad pony, this is several steps further down the curve of insanity. Quite honestly, you'd have to cut this entirely. We simply do not allow for characters with multiple personalities, as this easily results in the player choosing a personality based on their own feelings, rather then the character's.
We also do not allow adult flank-flanks, eyes changing color, or similar. We also like to have a more detailed story for the cutiemark, similar to the ones told by the mane six during their episode.
